Volunteer Application TEACH PROGRAMSince 1997, Give Every Child A Chance has provided free tutoring to students K-12th grade in their community. In order to qualify, the student must show academic need through report cards or MAP/CAASP test scores. A qualifying grade for tutoring is a C- or S-. Tutors will focus on one subject only. *Students may attend any school for tutoring with exception to our Tracy sites. Tutoring Application WE RISE, BY LIFTING OTHERSWHAT HAPPENS AT PROGRAM9HR. DAY BEFORE/AFTER SCHOOL PROGRAM Academics Staff encourages students to be as independent as possible and responsible for getting their work done. Students work in a group setting with staff (in some cases volunteers) offering guidance when requested. We do not guarantee all homework will be completed. Enrichment Activities may include arts & crafts, games, science, technology, engineering, and math (S.T.E.M.) projects, health education, field trips, guest speakers/instructors, drama productions, music, and more! Physical Education Activities can include basketball, low-organized games, free play, clinics, tournaments, exercises, etc. All recreation and Physical Education activities are aligned with the California State Standards. Student Pickup Authorization (SPA) Form-----IMPORTANT: THIS IS NOT ENROLLMENT-----This form is used to authorize designated individuals to pick up a student from our program. It ensures the safety and security of students by providing our organization with a record of approved guardians or caregivers. The form includes details such as the student's name, grade, authorized person's name, relationship to the student, and contact information. It must be signed by the parent or legal guardian.
